摘    要:目的 探讨老年高血压的动态血压特点。方法 采用无创动态血压仪监测并比较12例正常老年对照组、51例非老年高血压组、52例老年高血压组的ABPM各参数。结果 老年高血压组24h、白天、夜间SBP、DBP均值除白天DBP外,均显著高于老年对照组(P<0.05,P<0.01,P<0.001)。血压负荷值老年高血压组24h、白天、夜间SBP显著增高(P<0.01,P<0.001),DBP无差异。老年高血压组呈现夜间SBP下降率减小(P<0.05)。老年高血压与非老年高血压组比较,前者DBP均值及负荷值明显降低(P<0.01,P<0.001),而SBP两组无差异。各期老年高血压比较除白天SBP负荷值有差异外(P<0.05),余参数均无差异。结论 老年高血压主要表现为收缩期高血压,并伴有夜间血压下降率的减小。

Abstract:Objective To evaluate the feature of ambulatory blood pressure monitoring(ABPM) in elderly hypertensions. Methods Noninvasive ambulatory blood pressure recorder was used to carry out 24-hour ABPM in 52 elderly hypertensive patients and 51 youngerly hypertensive patients and 12 elderly controls. Results 24-hour mean SBP and DBP,daytime mean SBP,nighttime mean SBP and DBF were significantly higher in elderly hypertensive group than those in control group(P<0.05,P<0.01,P<0.001). SBP loads of elderly hypertensive group were higher than those of control group in 24-hour, daytime and nighttime (.P < 0.01,P<0.001). No difference was observed in DBP load. Nocturnal drop rate in blood pressure was less in elderly hypertensive group than in controls group(P<0.05). Elderly hypertensions displaid siglificantly lower mean DBP and DBP load at 24-hour, daytime and nighttime(P < 0.01, P < 0.001, P < 0.001). Except to daytime SBP,no differences were observed in different stages. Conclusion Elderly hypertension mainly displays systolic hypertension,and less nocturnal drop rate in blood pressure.
