血液灌流联合血液透析治疗急性肾衰竭的临床研究

目的探讨血液灌流(HP)联合血液透析(HD)治疗急性肾衰竭的临床效果及其价值。方法回顾性分析2007年12月至2011年12月山东省莱芜市人民医院收治的180例急性肾衰竭患者的临床资料,随机分为对照组和观察组,各90例,对照组运用HP的方法治疗,观察组运用HP+HD结合的方法进行治疗。比较两组治疗前后血液中的尿素氮(BUN)、血清肌酐(SCR)、肌酐(CR)、尿蛋白、中毒至治疗时间、住院天数以及肾功能恢复时间。结果治疗后两组血液中的BUN、CR、SCR、尿蛋白等指标比较差异有统计学意义(P<0.05);两组在中毒至治疗时间与住院天数方面比较差异无统计学意义(P>0.05),但观察组的肾功能恢复时间显著少于对照组(P<0.01)。结论将HP联合HD用于治疗急性肾衰竭,其临床效果尤佳,且大大缩短了肾功能恢复时间,应在急性肾衰竭临床诊治过程中加以推广应用。

Clinical Study on Effect of Hemoperfusion Combined with Hemodialysis for Acute Renal Failure

Objective To investigate the clinical effect and the value of hemoperfusion(HP) combined with hemodialysis(HD) in the treatment of acute renal failure.Methods Retrospective analysis on clinical data of 180 cases of acute renal failure patients from Shandong Province Laiwu City People's Hospital from December 2007 to December 2011 was done,the 180 cases were randomly divided into control group and observation group: 90 cases of the control group were treated by HP treatment,90 cases of the observation group were treated by HP + HD.The blood urea nitrogen(BUN),serum creatinine(SCR),urine protein,poisoning and treatment time,hospitalization days and renal recovery time of the two groups before and after treatment were compared.Results The blood biochemical detection indexes such as BUN,CR,SCR,urine protein of the observation group and the control group after treatment had statistically significant difference(P < 0.05);there was no statically significant difference in poisoning to treatment time and hospitalization days between the two groups(P > 0.05),but renal recovery time of the observation group was obviously shorter than the control group(P < 0.01).Conclusion HP joint HD as the treatment of acute renal failure has excellent clinical effect and greatly shortened renal recovery time,which should be applied and promoted in the clinical diagnosis and treatment.
