双侧去骨瓣减压术治疗颅脑损伤的临床研究进展

重型颅脑损伤继发的顽固性颅内压增高,传统的药物、物理治疗方案无确切疗效。去骨瓣减压术能显著降低颅内压,提高脑灌注压和氧分压。双侧去骨瓣减压术(BDC)作为对传统去骨瓣减压术的扩展与补充,在救治难控性颅内压增高的颅脑损伤患者中具有重要作用。但是,BDC的适应证、并发症以及能否改善重型颅脑损伤患者的远期预后仍存在争论。

Clinical Research Progress in Bilateral Decompressive Craniectomy Treatment for Traumatic Brain Injury

Intracranial hypertension after sever traumatic brain injury often fails to respond to traditional strategies such as drugs or psychical methods.Decompressive craniectomy can apparently decrease intracranial pressure,improve cerebral perfusion pressure and increase partial pressure of oxygen in brain tissue.As its extension and supplement,bilateral decompressive craniectomy plays an important role in treating intractable intracranial hypertension.Yet,arguments about its indications,complications and long-term prognosis are still going on.
