多节段脊柱骨折的治疗体会

目的探讨多节段脊柱骨折的临床特点及诊治体会,为临床提供参考。方法回顾性分析2008年1月至2011年8月深圳市宝安区龙华人民医院收治的169例脊柱骨折患者的临床资料,其中多节段脊柱骨折58例作为多节组,单节段脊柱骨折111例作为单节组,比较两组患者的临床特点、诊治及预后等情况。结果两组患者主要受伤原因、受伤部位、损伤严重度评分法(ISS评分)及治疗情况进行比较,差异有统计学意义(P<0.05)。所有患者治疗1年后进行随访,对两组患者治疗前和治疗后脊髓损伤程度ASIA分级进行比较,除D级和E级外,差异均有统计学意义(P<0.05)。结论多发性脊柱骨折临床伤情危重,多伴有脊髓损伤,合并伤较多,积极手术治疗有利于促进康复。

The Treatment Experience on Multiple-level Spinal Fracture

Objective To explore clinical features and treatment experience of multiple-level spinal fracture,to provide reference for clinical.Methods Clinical data of 169 cases of the hospitalized patients with spinal fractures at Shenzhen Bao' an District Longhua People' s Hospital from January 2008 to August 2011 were retrospective analyzed: 58 cases with multiple-level spinal fracture were as the multisection group,111 cases of single segmental spinal fracture were as single section group,and the clinical characteristics,diagnosis,treatment and prognosis of the two groups were compared.Results The main injury causes,injury parts and injury severity score(ISS score) method and treatment conditions of the two groups were compared,the difference was statistically significant(P < 0.05).All patients were followed up 1 year later,the injury of spinal cord(ASIA) of the two groups before and after treatment were compared,except grade D and grade E,all differences had statistical significance(P < 0.05).Conclusion Multiple-level spinal fracture is severe in clinical,often accompanied by spinal cord injury,and merged injuries.Active surgical treatment can promote the rehabilitation.
