治疗性沟通对维持性血液透析患者希望水平的影响

目的 评价治疗性沟通对维持性血液透析患者希望水平的影响.方法 选择维持性血液透析患者86例,采用随机数字表法分为对照组和实验组,各43例.对照组实施常规护理,实验组在常规护理的基础上,采用治疗性沟通模式.两组均在干预前及干预2个月后,采用焦虑自评量表(SAS)及抑郁自评量表(SDS)进行焦虑抑郁情绪评定;采用希望水平量表(HHI)进行希望水平评定.结果 干预前,两组各项指标差异均无统计学意义(P＞ 0.05).干预后,实验组SAS评分(47.36±6.12)分,对照组(55.33±5.24)分,实验组焦虑水平低于对照组,差异有统计学意义(P＜0.05 l实验组SDS评分(45.27±6.58)分,对照组(56.56±5.89)分,实验组抑郁水平比对照组低,差异有统计学意义(P＜0.05);实验组希望水平评分(36.64±6.68)分,对照组(28.56±7.82)分,希望水平增高,实验组好于对照组,差异有统计学意义(P＜0.05).结论 在维持性血液透析患者的临床护理工作中,将治疗性沟通的护理方法运用其中,可缓解患者的焦虑、抑郁情绪,提高希望水平.

Influence of therapeutic communication on level of hope in patients undergoing maintenance hemodialysis

Objective To evaluate the influence of therapeutic communication on the level of hope in patients undergoing maintenance hemodialysis.Methods 86 patients undergoing maintenance hemodialysis were selected and randomly divided into a control group and an experimental group, 43 for each group.The control group were routinely cared;in addition, the experimental group were intervened with therapeutic communication.Both groups were evaluated by self rating anxiety scale (SAS) and self rating depression scale (SDS) before and 2 months after the intervention.Results There were no statistical differences in all the indexes between these two groups before the intervention (P＞0.05).After the intervention, the scores of SAS and SDS were (47.36±6.12) and (45.27±6.58) in the experimental group and were (55.33±5.24) and (56.56±5.89) in the control group;the levels of anxiety and depression were lower in the experimental group than in the control group, with statistical differences (P＞0.05);the level of hope was (36.64±6.68) in the experimental group and was (28.56±7.82) in the control group, with a statistical difference(P＜0.05).Conclusions Therapeutic communication in the nursing care for patients undergoing maintenance hemodialysis can ease their anxiety and depression and improve their level of hope.
