右美托咪定对妇科患者腹腔镜术后镇痛效果的研究

目的:观察右美托咪定(dexmedetomidine,DEX)用于妇科腹腔镜术后患者静脉自控镇痛(patient controlled intravenous analgesia,PCIA)的止痛效果。方法:选取40例择期妇科腹腔镜手术患者,随机分为DEX组(D组)和芬太尼组(F组),每组各20例。比较两组术后6 h在视觉模拟量表(VAS)上的平均疼痛评分、Ramsay镇静评分、术后恶心呕吐(PONV)发生率、疼痛控制满意度和术后胃肠功能恢复。结果:术后6 h,两组患者平均VAS评分差异无统计学意义(P>0.05);D组PONV发生率低于F组(P<0.05);两组间Ramsay镇静评分差异无统计学意义(P>0.05);D组患者对术后镇痛的满意度较高,且术后胃肠功能恢复所用时间较F组少(P<0.05)。结论:妇科腹腔镜术后患者行单药右美托咪定静脉给药的镇痛方法有效,且该镇痛方法可提高患者术后镇痛满意度,缩短术后胃肠功能恢复时间。

Study on the postoperative analgesia effect in gynecologic patients undergoing laparoscopic surgery

Objective:To observe the analgesic effect of dexmedetomidine (DEX) for patient controlled intravenousanalgesia (PCIA) after gynecological laparoscopic surgery.Methods:40 cases of female patients undergoing gynecological laparoscopic surgery were randomly divided into DEX group (group D) and Fentanyl group (group F) with 20 cases in each group.The average pain score,the Ramsay sedation score,the incidence of postoperative nausea and vomiting (PONV),the pain control satisfaction and the postoperative gastrointestinal function recovery were observed on the visual analogue scale (VAS) at 6 h after operation were compared in the two groups.Results:After operation,the average VAS score of the two groups had no statistical significance (P ＞ 0.05),the incidence rate of PONV in group D was lower than that in group F (P ＜ 0.05),there were no significant differences in Ramsay sedation score between the two groups (P ＞ 0.05).The patients in group D had higher satisfaction for postoperative analgesia than the patients in group F,and the time of postoperative recovery of gastrointestinal function in the group D was less than that in the group F (P ＜0.05).Conclusion:The analgesic effect of intravenous administration of single drug dexmedetomidine on postoperative analgesia in gynecologic laparoscopic surgery is effective,and the analgesic method can improve the postoperative analgesia satisfaction and shorten the recovery time of postoperative gastrointestinal function.
