氯沙坦钾对老年原发性高血压伴高尿酸血症的临床疗效及安全性评价

目的:观察氯沙坦钾治疗原发性高血压合并高尿酸血症的临床疗效。方法:分析88例原发性高血压合并高尿酸血症的患者资料,依据治疗方法的不同将患者分为观察组(氯沙坦钾治疗,n=45)及对照组(卡托普利治疗,n=43),治疗6个月后观察降压效果,对比两组患者血尿酸及外周血炎症因子的改变。结果:与治疗前比较,两组患者收缩压及舒张压均显著降低(P<0.05);观察组治疗期间痛风累积发生率显著低于对照组(11.11%vs.27.91%;P=0.043);观察组患者治疗后血尿酸、IL-6及TNF-α水平均显著降低,且均低于对照组(P<0.05)。观察组患者治疗后血尿酸水平与IL-6(r=0.897,P<0.01)及TNF-α(r=0.884,P<0.01)水平均呈现显著正相关关系。结论:原发性高血压合并高尿酸血症的治疗中,氯沙坦钾显著优于卡托普利片。

The clinical efficacy and safety evaluation of losartan potassium in elderly patients with primary hypertension accompanied by hyperuricemia

Objective:To analyze the clinical efficacy of losartan potassium in elderly patients with primary hypertension accompanied by hyperuricemia.Methods:Clinical data of 88 cases of patients with primary hypertension associated with hyperuricemia were analyzed.Patients were divided into the observation group (Losartan potassium,n =45) and the control group(Captopril,n =43) by therapeutic methods,to observe the antihypertensive effect after 6 months of treatment,and to contrast the change in the two groups of blood uric acid and inflammatory factors.Results:The systolic blood pressure and diastolic blood pressure decreased prominently in both groups after treatment (P ＜ 0.05).In the treatment period,the cumulative incidence of gout in the observation group was lower than that in the control group (11.11％ vs.27.91％,P =0.043).After treatment,The blood uric acid,IL-6 and TNF-α levels in the observation group significantly reduced,and were lower than those in the control group(P ＜ 0.05).The level of blood uric acid in the observation group was significantly positively correlated with IL-6 (r =0.897,P ＜ 0.01) and TNF-α (r =0.884,P ＜ 0.01).Conclusion:In the treatment of primary hypertension associated with hyperuricemia,Losartan potassium showed a better clinical efficacy than Captopril.
