胸水CEA、CA125和CYFRA21-1检测对肺癌胸膜转移的诊断价值

摘    要:目的探讨胸水中癌胚抗原(CEA)、糖类抗原125(CA125)、细胞角蛋白19片段(CYFRA21-1)检测对肺癌胸膜转移的诊断价值。方法采用放射免疫分析法对48例肺癌患者胸水、血清及35例非肺癌患者胸水进行了免疫蛋白定量分析。结果肺癌组胸水中3项指标水平均明显高于非肺癌组水平(P<0.01);肺癌组胸水中3项指标水平明显高于血清中水平(P<0.01);CEA、CA125及CYFRA21-1联合检测其敏感性和特异性高达96.01%和96.11%。结论胸水CEA、CA125和CYFRA21-1检测对肺癌胸膜转移诊断具有重要临床意义。

Abstract:Objective To study the diagnostic value of detection of CEA,CA125 and CYFRA21-1 in pleural fluid samples for pleural metastasis of lung cancer.Methods Immunoprotein quantity of CEA,CA125 and CYFRA21-1 was analyzed in pleural fluid and serum from patients with lung cancer(48 caces) and in pleural fluid from non cancerous patients(35caces) by using commercially available radioimmunoassay.Results The levels of CEA,CA125 and CYFRA21-1 in pleural fluid of patients with lung cancer were significantly high than those of non cancerous patients(P<0.01).The levels of CEA,CA125 and CYFRA21-1 in pleural fluid were obviously higher than those in serum(P<0.01).The sensitivity and the specificity of CEA CA125 CYFRA21-1 were 96.01% and 96.11% respectively.Conclusions Detection of CEA,CA125 and CYFRA21-1 in pleural fluid might be helpful for diagnosing pleural metastasis of lung cancer.
