随机尿白蛋白／肌酐比在早期糖尿病肾损害中的临床应用评价

目的评价随机尿白蛋白／肌酐检测与24h尿蛋白定量是否能等效。方法选取确诊糖尿病患者110例和健康对照50例，对其随机尿P／C与24h尿蛋白定量进行相关分析；比较糖尿病组和正常对照组中微量白蛋白尿和蛋白尿的阳性率。结果糖尿病患者的自蛋白肌酐比的增加与滤过功能下降有关，不同肾功能分期中，随机尿UmAlb／UCr与24hUp定量保持良好的相关性，整体的相关系数0．96。但与24hUmAlb定量相比，随机尿Urn—Alb／UCr有一定的假阳性。结论尿蛋白检测的影响因素较多，随机尿UmAlb／UCr作为筛查实验有明显的优势，但确诊仍以24hUp定量为佳。

Clinical evaluate application of albumin to creatinine ratio in early diabetic nephropathy

Objective To observe could UmAlb/UCr in random urine specimem were equivalent for urinary protein excretion in 24 hours urine specimem in early diabetic nephropathy. Methods 24 h urine collection and random urine specimen were colletted from 110 diabetic patients and 50 health control. The correlion between UmAlb/UCr in random urine specimere and 24 h Up （urine protein）were analyzed the correlations between UmAlb/UCr in random urine specimens and urinary protein excretion in 24 hours collections were analyzed. To compare the positive ratio in diabet group and control group. Resuits The value of UmAlb/UCr in random urine has negative correlations with GFR, while UmAlb/UCr in random urine specimem and 24 h Up keep quite well positive correlations. Compared with 24 h Up, UmAlb/UCr in random urine has some false positive. Conclusion UmAIb/UCr in random urine has certain advantage for scanning nephritic damage, but 24 h Up is better for diagnosis of nephropathy.
