四妙糖足康治疗糖尿病足临床观察

目的观察四妙糖足康治疗糖尿病足的临床疗效。方法75例糖尿病足患者随机分为2组,在西药常规治疗基础上,治疗组予四妙糖足康基本方为主随症加味,对照组予盐酸川芎嗪注射液静脉滴注。2组均以4周为1个疗程,观察2组临床疗效,糖尿病足溃疡大小、脓性分泌物情况、创面肉芽情况及Wagner分级变化,胫后神经传导速度、足背动脉搏动情况及血流速度,空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)、丙氨酸氨基转移酶(ALT)、血肌酐(Cr)、甘油三酯(TG)、总胆固醇(TC)、尿白蛋白排泄率(UAER)。结果治疗组总有效率83.72%,对照组总有效率68.75%,2组总有效率比较差异有统计学意义(P0.05),治疗组疗效优于对照组;治疗组糖尿病足溃疡大小、脓性分泌物情况、创面肉芽情况、Wagner分级及胫后神经传导速度、足背动脉搏动情况、血流速度好转情况均优于对照组(P0.05)。2组治疗后FPG、2hPG、HbAlC、ALT、Cr、TG、TC及UAER比较差异无统计学意义(P0.05)。结论在西医常规治疗基础上,四妙糖足康有益气养阴,活血化瘀功效,治疗糖尿病足疗效确切。

Clinical observation of Simiaotangzukang on diabetic foot

Objective To observe the effect of Simiaotangzukang on diabetic foot.Methods 75 patients with diabetic foot were randomly divided into two groups. Patients in control group （n=32） were treated by Ligustrazine Hydrochloride Injection. 43 cases in treatment group were treated by combination of Simiaotangzukang and routine therapy. The course was four weeks in two groups. The clinical effect including area of ulcer of foot, purulent secretion, granulation wound, Wagner grade, conduction velocity of posterior tibial nerve, arteria dorsalis pedis impulse, blood flow rate, FPG, 2hPG, HbAlc, ALT, Cr, TG, TC and UAER were measured. Results The total effective rate in treatment group （83.72%） was higher that in control group （68.75%, P〈0.05）. The improvement of area of ulcer of foot, purulent secretion, granulation wound, Wagner grade, conduction velocity of posterior tibial nerve, arteria dorsalis pedis impulse and blood flow rate in treatment group were superior to those in control group （P〈0.05）. There was not significant difference between two groups on FPG, 2hPG, HbAlc, ALT, Cr, TG, TC and UAER after treatment.Conclusion On the basis of routine therapy, Simiaotangzukang and has supplementing qi and nourishing yin, activating blood and dissolving stasis, and definite effect on diabetic foot.
