急性阑尾炎腹腔镜手术与开腹手术对比分析

目的：探讨急性阑尾炎腹腔镜手术与开腹手术的疗效。方法选择100例急性阑尾炎患者，分别采用腹腔镜手术（腹腔镜组，n=50）与传统开腹手术（开腹组，n=50）。结果两组手术时间、术中出血量比较无明显差异（P＞0.05），但腹腔镜组胃肠功能术后恢复的时间明显短于开腹组（P＜0.05）。腹腔镜组镇痛药使用率为4%（2/50），对照组为22%（11/50）；腹腔镜组无并发症发生，开腹组出现切口感染5例，腹腔脓肿2例，两组并发症发生率比较，差异有统计学意义（0 vs 14%，P＜0.05）。结论腹腔镜手术可明显降低术后切口感染率、胃肠功能恢复快、创伤较小，是一项有效、安全的术式，值得推广和应用。

Comparative analysis on treatment of acute appendicitis by laparoscopic and laparotomy

Objective To study the clinical effect of laparoscopic and laparotomy in the treatment of acute appendicitis. Methods 100 patients with acute appendicitis were divided into laparoscopic group and laparotomy group,with 50 cases in each group. Results The operation time and the intraoperative blood loss of the two group had no significant differences（P＆gt;0.05）.The recovery time of postoperative gastrointestinal function in laparoscopic group was shorter than laparotomy group（P 〈 0.05）.The use rate of analgesics of laparoscopic group was 4%（2/50） with non-complication, while in laparotomy group,the use rate of analgesics was 22%（11/50）,there were 5 cases with incision infection and 2 cases with abdominal abscess.The different of the complication rate between the two group had statistical significance（0vs 14%,P 〈 0.05）. Conclusion Laparoscopic surgery has lower rate of incision infection,rapid restoration of gastrointestinal function,less traumatic,is an effective and safety operative method, worthy to be popularized and applied.
