胰岛素抵抗与糖尿病肾病

糖尿病肾病发病与高血糖、胰岛素抵抗、炎性反应有关。目前认为,肥胖、胰岛素抵抗以及炎性反应之间具有相关性。肥胖和高脂饮食激活脂肪细胞、肝细胞和巨噬细胞内的核因子κB抑制物激酶β/核因子κB和c-Jun氨基末端激酶信号途径。肥胖激活核因子κB抑制物激酶β导致核因子κB移位和大量炎性因子的表达,从而导致胰岛素抵抗。胰岛素抵抗和全身炎性反应共同在糖尿病肾病的发病中起重要作用。而针对改善胰岛素抵抗和抑制全身炎性反应的措施已成为治疗糖尿病肾病的新方法。

Insulin Resistance and Diabetic Nephropathy

The pathogenesis of diabetic nephropathy is associated with hyperglycemia,insulin resistance and inflammation.It is known that there is dependability among obesity,insulin resistance and inflammation.Obesity and high fat diet can IKKβ/NF-κB and JNK pathway in adipocyte,hepatocyte and macrophage.Obesity can result in NFκB aversion and expression of inflammatory factor by activating IKKβ,which will then result in insulin resistance.Insulin resistance and systemic inflammatory reaction are all play an important role in the genesis of diabetic nephropathy.The measurements aiming at promoting insulin resistance and inhibiting systemic inflammatory reaction will be the new methods to treat diabetes.
