下腹正中切口开放式腹膜前修补术治疗腹股沟复发疝的临床研究

目的 探讨下腹正中切口开放式腹膜前修补术治疗腹股沟复发疝的安全性及有效性.方法 回顾性分析200r7年1月至2010年1月腹股沟复发疝患者48例临床资料,其中行下腹正中切口开放式腹膜前修补术(实验组)28例,前入路修补术(对照组)20例.对阴囊血肿、缺血性睾丸炎、平均手术时间、术后疼痛、异物感、复发进行研究.结果 经统计学分析,二组在平均手术时间、术后复发率、阴囊血肿等方面差异均无统计学意义(P>0.05),在缺血性睾丸炎、术后疼痛、异物感方面差异有统计学意义(P<0.05).结论 开放式腹膜前修补术治疗腹股沟复发疝可明显减少各种并发症和术后不适的发生率,疗效肯定,值得临床推广应用.

Clinical research of open preperitoneal approach through inferior abdominal median incision for recurrent inguinal hernia repair

Objective To investigate the safety and effectiveness for the open preperitoneal approach through inferior abdominal median incision in treatment of recurrent inguinal hernia. Methods The data from forty-eight patients with recurrent inguinal hernia treated by tension less hernia repair between January 2007 and January 2010 in our hospital were analyzed. Of all 28 cases were treated by open preperitoneal approach through inferior abdominal median incision as thetest group,the others were treated by lichtensteins surgery as thecontrol group. The difference of scrotal hematoma,ischemicorchitis,operation time,postoperative pain,postoperative foreign body sensation,and the recurrence rate were compared. Results There was no significantly different between the results from test group and control group for the operation time,recurrence rate,and scrotal hematoma,but the incidences of scrotal hematoma,ischemic orchitis, postoperative pain,postoperative foreign body sensation. Conclusions The open preperitoneal approach through inferior abdominal median incision for recurrent inguinal hernia repair can significantly reduce the incidence of complication and postoperative discomfort with the value of clinical application.
