改良经腹腔腹膜前腹腔镜疝修补术与开放式无张力疝修补术临床疗效比较

目的 分析比较改良经腹腔腹膜前腹腔镜疝修补术(TAPP)与开放式无张力疝修补术治疗成人腹股沟疝的临床疗效.方法 回顾性分析60例改良TAPP疝修补术与50例开放式无张力疝修补术患者的临床资料,对手术时间、住院时间、住院费用、术中出血量及术后并发症发生率等进行比较.结果 二组患者均随访18个月以上,腹腔镜手术组术后出现血清肿4例、神经痛3例、伤口感染1例,复发1例;开放手术组术后发生血清肿3例、神经痛2例,复发2例.二组平均手术时间、并发症发生率、住院费用和复发率比较,差异无统计学意义(P>0.05);平均出血量、住院时间二组比较差异有统计学意义(P<0.05).结论 改良TAPP术具有创伤小、住院时间短、康复快、美容、复发率低等优点,是目前成人腹股沟疝无张力修补术中较为理想的术式.

Laparoscopic herniorrhaphy with Open tension-free herniorrhaphy for adult ingiiiiml hernia

Objective To compare the effect of laparoscopic and open surgery for adults tension-free hernia repair. Methods A total of 60 cases of adult laparoscopic hernia repair and 50 cases of open surgery were retrospectively analyzed and compared for the operation time,hospitalization stay,cost,bleeding and post-operation complications. Results After 18-month follow-up,4 cases of seromas,3 cases of neuropathic pain,one case of infection,and one case of recovery were observed in the group of laparoscopic hernia repair. There were 3 cases of seromas,2 cases of neuropathic pain,and 2 cases of recovery in the group of open surgery. The mean operating time,the incidence of complications and hospitalization costs and relatively relapse were compared without significant difference (P > 0. 05) . The mean amount of bleeding and length of hospitalization stay were compared with significant difference (P < 0. 05) . Conclusions Laparoscopic surgery has less trauma with shorter hospital stay,faster recovery,and low relapse rate,which is ideal for the tension-free hernia repair in the adult inguinal herina.
