慢性乙肝患者门静脉血流动力学与肝纤维化关系

摘    要:目的 观察慢性乙肝患者门静脉血流动力学与肝纤维化的关系。方法 测定102例慢性乙型肝炎患者血清纤维化指标,多普勒超声检测门静脉内径、门静脉主干血流速度、门静脉血流量,并观察肝组织的病理改变情况。结果 门静脉血流速度与肝纤维化分期关系密切,随着肝纤维化程度的发展而降低,肝纤维化程度严重者,门静脉血流速度明显低于肝纤维化程度轻者。在肝纤维化分期为S4时较S1时减慢了28.4%,(P〈0.01)。门静脉血流速度与血清透明质酸(HA)、层粘蛋白(LN)、Ⅳ型胶原(CIV)相关。门静脉血流量与肝纤维化分期无关。结论 门静脉血流速度与肝纤维化分期关系密切,随着肝纤维化程度的发展而降低;而门静脉血流量与肝纤维化分期无相关性。

Abstract:Objective To investigate the relations between the portal parameters of homodynamic and stages of liver fibrosis of chronic hepatic B patients.Methods Fibrosis parameters of blood serum of one hundred and two patients with chronic viral hepatic B were measured with Doppler ultrasonic technique.The inner diameters,blood flow velocity,blood flow volume of their portal veins were investigated.And,the pathological changes of liver tissues were examined.Results The blood flow velocity of portal vein correlates very well with the stages of liver fibrosis,and reduces with the development of the degree of liver fibrosis.The blood flow velocity of severe fibrosis patients is obviously smaller than that of mild patients,and is 28.4% slower compared stage 4 with stage 1(P<0.01).The blood flow velocity is associated with hyaluronic acid (HA) and laminin (LN) and collagen IV (CIV) of blood serum,However,the blood flow volume is not related with the stages of hepatic fibrosis.Conclusions The blood flow velocity of portal vein relates with the stages of liver fibrosis,and reduces with the stages of liver fibrosis.However,the blood flow volume is not associated with the stages of hepatic fibrosis.
