血小板计数与慢性肝炎纤维化程度和合并脾亢的关系

目的了解慢性乙型肝炎(简称乙肝)患者血小板(PLT)计数变化与慢性肝炎肝纤维化程度的关系,以及与肝纤维化合并脾亢进展的关系。方法选取慢性乙型病毒性肝炎患者200例行肝组织病理学检测了解肝组织纤维化程度,全自动血细胞分析仪检测慢性乙型肝炎患者外周血的血小板计数,并比较肝纤维化合并脾亢与肝纤维化严重程度的关系,以及比较血小板计数与肝纤维化脾亢进展的关系。结果平均PLT计数随纤维化严重程度的进展呈下降趋势,其计数值在肝纤维化各分期间差异有统计学意义(P0.05);平均PLT计数并随脾亢的进展也呈下降趋势。结论慢性乙肝患者在缺乏肝组织病理学检查时PLT计数可作为一项无创性的客观指标用于临床对肝纤维化严重程度的评估和辅助诊断,并且PLT计数可在一定程度上反映肝纤维化合并脾亢的严重程度。

Relationship between platelet count and chronic hepatitis fibrosis complicating hypersplenism

Objective To evaluate the relationships between platelet count（PLT）and live fibrosis either complicated with or without hypersplenism.Methods 200 patients with chronic hepatitis B were recruited into the present study,the severity of liver fibrosis was evaluated through pathological examination,and the automated blood cell analyzer was employed for the PLT count in peripheral blood.To compare the relationships between either liver fibrosis combined with hypersplenism and severity of liver fibrosis or the PLT count and the progression of liver fibrosis compilicated with hypersplenism.Results Mean PLT count decreased gradually with the progressions of both liver fibrosis and hypersplenism,moreover,PLT count was found to be significantly different between each phase of liver fibrosis（P〈0.05）.Conclusion PLT count can be taken as a non-invasive and objective index for both the clinical evaluation of the severity and the auxiliary diagnosis of liver fibrosis when the pathological examination of the liver tissues is unavailable.Moreover,PLT count can reflect to some extent the severity of liver fibrosis complicated with hypersplenism.
