四肢骨肉瘤新辅助化疗后影像学表现与大体标本的相关性研究

摘    要:目的探讨四肢髓内型骨肉瘤新辅助化疗后影像学表现与大体标本的相关性。方法选取我科2005年9月-2006年12月四肢髓内型骨肉瘤17例,经全身大剂量三疗程化疗后,常规行术前MRI平扫,术后取沿冠状面剖开的1/2大体标本固定备用,再对留取标本行DR及CT检查,通过分区逐一对比影像学与病理表现。结果X线、CT与病理检查之间差异显著(P〈0.05);术前MRI平扫检查与病理检查之间无显著性差异(P〉0.05)。结论MRI检查骨肉瘤髓内侵润相对误差较小,依据MRI测量结果扩大切除2-3cm是安全的截骨范围。

Abstract:Objective To study the correlation between imaging manifestations of intramedullary osteosarcoma of extremities after neoadjuvant chemotherapy and gross specimens. Methods MRI was performed after three courses of large dosage chemotherapy for 17 patients with intrameduUary osteosarcoma of extremities admitted to our department from September 2005 to December 2006. Gross specimens were taken by cutting along the coronal axis and stored in 10% formalin for X-ray and CT examination. Imaging manifestations and pathological findings of the specimens were compared. Results X-ray, CT and pathological examinations showed a difference in length of osteosarcoma(P〈0.05). However, MRI and pathological examination showed no difference in length of osteosarcoma before operation(P〉0.05). Conclusion Osteotomy can be performed 2 to 3cm beyond the margin since MRI shows a relatively small error of the invasion of osteosarcoma.
