电视胸腔镜与腋下小切口肺叶切除术比较

目的比较电视胸腔镜（VATS）和腋下小切口（LAT）两种微创手术行肺叶切除术的优缺点。方法回顾性分析2002年6月－2009年12月通过两种微创术式进行解剖学肺叶切除术的临床资料。结果采用VATS手术107例，LAT手术135例；病例包括原发性肺癌、肺转移瘤、炎性假瘤、肺隔离症等，均行肺叶切除术。两组在手术出血量、胸腔引流管放置时间、术后住院时间以及并发症发生率上无统计学差异，LAT组的手术时间略短，VATS组术后当天胸腔引流量略少。结论两种微创手术行肺叶切除术无显著差别，各有其优势。

Video-assisted thoracoscopic surgery and limited axillary thoracotomy in lobectomy: A comparative study

Objective To compare the advantages and disadvantages of video-assisted thoracoscopic surgery（VATS） and limited axillary thoracotomy（LAT） in lobectomy. Methods Clinical data about VATS and LAT in anatomical lobectomy performed from June 2002 to December 2009 were retrospectively analyzed. Results VATS and LAT were performed in lobectomy for primary lung cancer, lung metastatic tumor, inflammatory pseudotumor, pulmonary sequestration in 107 and 135 patients, respectively. No significant difference was observed in blood loss, drainage time, hospital stay time, and complications between the two groups. However, the operation time was shorter and the pleural drainage volume was slightly less in patients undergoing VATS than in those undergoing LAT. Conclusion Each of the two minimally invasive surgical procedures has its own advantages in anatomical lobectomy.
