冠心病患者血浆炎性细胞因子及P-选择素的临床价值

目的检测冠心病患者血浆中白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)及P-选择素的变化,并探讨其在冠心病发病过程中的机制。方法选择住院冠心病患者50例,分为不稳定性心绞痛(UA)患者组32例,稳定性心绞痛(SA)患者组18例,另选20例健康人作为对照组。分别测定其血浆中IL-6、IL-8、TNF-α及P-选择素水平,并进行了比较;同时分析P-选择素与IL-6、IL-8、TNFα-的相关性。结果 IL-6、IL-8、TNF-α及P-选择素水平,UA患者组、SA患者组均高于健康对照组,差异有统计学意义(P0.01),UA患者组明显高于SA患者组(P0.01)。血浆P-选择素水平与IL-6、IL-8、TNF-α水平呈正相关(均P0.01)。结论检测冠心病患者血浆中IL-6、IL-8、TNF-α及P-选择素水平,对冠心病患者病程判断、评价、预后有重要参考价值。

Clinical value of plasma inflammatory cytokines and P-selection in patients with coronary heart disease

Objective To detect plasma interleukin-6（IL-6）,interleukin-8（IL-8）,tumor necrosis factor-α（TNF-α）and P-selection in coronary heart disease and to explore the possible mechanism in the pathogenisis of coronary heart disease.Methods 50 patients hospitalized with coronary heart disease,were divided into two groups,32 cases with unstable angina（UA）and the other 18 cases with stable angina（SA）.Then other 20 healthy people were chosen as the control group.Their plasma IL-6,IL-8,TNF-α and the level of P-selection were measured and compared.The correlation of P-selectin with IL-6,IL-8 anf TNF-α was also analyzedResults Plasma IL6,IL8,TNF-α and P-selectin levels of UA and SA groups were higher than those of the healthy control group with significant differences（P 〈0.01）.Plasma IL-6,IL-8,TNF-α and P-selectin levels of UA group were significantly higher than those of the SA group（P〈0.01）.P-selectin levels in plasma and IL-6,IL-8,TNF-α levels were significantly positively correlated（all P〈0.01）.Conclusion The detection of IL-6,IL-8,TNF-α and P-selection levels of inpatients with coronary heart disease has important reference value in the judgment of the disease progress,evaluation and prognosis.
