经肝动脉联合经皮穿刺脾选择性门静脉化疗栓塞治疗中晚期肝癌

目的探讨经肝动脉联合经皮穿刺脾选择性门静脉化疗栓塞（TACE＋PTSPVCE）治疗中晚期肝癌的临床疗效。方法将32例无手术切除指征并有门静脉血供的中晚期肝癌患者分为两组,13例（TACE＋PTSPVCE组）施行TA-CE联合PTSPVCE治疗,19例（对照组）施行单纯TACE。全部病例随访11～37个月并比较两组的疗效。结果TACE＋PTSPVCE组和对照组的肿瘤缩小率分别为84.62%和63.16%;肝左叶体积增大率分别为38.43%、10.78%;1、2、3年的生存率分别为77.78%、55.56%、33.33%和58.82%、35.29%、17.65%,两者比较均差异有统计学意义（P〈0.05）。全部病例均未出现严重并发症。结论TACE＋PTSPVCE治疗中晚期肝癌的疗效优于单纯TACE;经皮穿刺脾门静脉插管是一种安全实用的门静脉插管方法。
