经胸彩色多普勒超声心动图在先天性左室-右房通道介入封堵术中的应用价值

目的探讨经胸彩色多普勒超声心动图在先天性左室-右房通道（LV—RA）介入封堵术治疗中的临床价值．方法23例IN-RA患者，术前行超声心动图检查，按缺损位置分为三尖瓣瓣上型、瓣环型和瓣下型；按分流结果分为完全分流型和部分分流型：而后行介入封堵术，术后即刻和术后7d复查超声心动图。结果23例LV—RA患者，三尖瓣辩上型8例，瓣环型8例，瓣下型7例；完全分流型8例，部分分流型15例。行介入封堵术成功的患者21例，失败2例；术后即刻行左心室、升主动脉造影和经胸彩色多普勒超声心动图检查，均无残余分流和加重的三尖瓣反流；术再7d复查超声心动图示无残余分流，无严重并发证。结论经胸彩色多普勒超声心动图在LV—RA患者术前分类和介入封堵术的指导方面起到重要辅助作用。

Application Value of transthoracic color Doppler echocardiography in interventional occlusion of congenital left ventricular-right atrium communication

Objective To evaluate the clinical application value of transthoraeic color Doppler eehoeardiography in interventional occlusion of congenital left ventricular- right atrium （ LV - RA） communication. Methods Twenty - three patie,Its with L＋V -RA were examined by echocardiography before operation, then divided into three types： above tricuspid, tricuspid annulus and below tricuspid according to the defect location, and divided into two types： complete shunt and part shunt according to the shunt results, Interventioual occlusion was pertormed, and echocardiography was performed again immediately and 7 days after treatment. Results In 23 patients with LV - RAC, there were 8 cases of above tricuspid, 8 cases of tricuspid annulus and 7 cases of below tricuspid, while there were 8 cases of complete shunt and 15 cases of part shunt, lnterventional ocelusion suecessed in 21 cases and failed in 2 cases. Left ventrieular, aortic augiography and transthoracie echocardiography were performed immediately after treatment, there was no residual shunt and aggravated tricuspid regurgitation. Echoeardiography was performed 7 davs after treatment, there was no residual shunt and serious complications. Conclusion Transthoracie color Doppler eehocardiography has significant value in preoperative classification and interventional occlusion in patients with LV - RA.
