中西医结合治疗急性重症胰腺炎并发急性呼吸窘迫综合征临床研究

目的:研究中药鼻饲对急性重症胰腺炎并发急性呼吸窘迫综合征的临床疗效。方法:选取符合急性重症胰腺炎并发急性呼吸窘迫综合征的患者38例,随机分为两组,治疗组18例,对照组20例;对照组给予常规西医治疗,治疗组在此基础上经鼻空肠管鼻饲中药(清胰汤合大黄牡丹汤加减),每日1剂早晚分服,连续7天。结果:治疗后两组比较平均机械通气天数、病死率、血淀粉酶及72 h后PaO2/FiO2与Qs/Qt等情况,治疗组疗效优于对照组(P<0.05)。结论:中药(清胰汤合大黄牡丹汤加减)对改善急性重症胰腺炎并发急性呼吸窘迫综合征的呼吸情况、血淀粉酶、降低患者死亡率有确切疗效,值得推广。

Severe Acute Pancreatitis Complicated with Acute Respiratory Distress Syndrome in Combination Therapy

Objective ： To study the effect of medicine nasal feeding in severe acute pancreatitis complicated with acute respira- tory distress syndrome. Methods：Consistent with acute pancreatitis complicated by acute respiratory distress syndrome,38 patients were randomly divided into two groups, the treatment group 18 patients and the control group 20 cases;the control group received conventional Western medicine and treatment group on the basis added nasal feeding medicine（ Qingyitang together with Rhubarb Peony Decoction） in the moming and evening,7 consecutive days. Results ： After treatment, compar!ng the average number of days of mechanical ventilation, mortality, serum amylase and 72 hours after PaOJFiO2 and Qs/Qt, etc. , the treatment group was better than the control group（P 〈 0. 05）. Conclusion：Chinese medicine（ Qingyitang together Rhubarb Peony Decoction）can improve se- vere acute pancreatitis complicated with acute respiratory distress syndrome. It is worthy of promoting.
