胰岛素与多囊卵巢综合征发病的关系

对30例LH或LH/FSH正常的多囊卵巢综合征患者进行糖耐量试验(OGTT)观察有关的内分泌变化,并以15名正常妇女作对照。结果:(1)所有受试者的糖耐量均在正常范围,但患者组在试验后30、60、90分钟点血糖水平明显高于对照组;(2)空腹及OGTT后各时点胰岛素水平患者组约为对照组2～3倍,而C-P肽亦以患者组为高;(3)OGTT后患者组睾酮增加约1.97nmol/L而对照组睾酮下降约(0.44nmol/L)。表明此类患者有高胰岛素血症,这与外周组织对胰岛素耐受及肝脏清除胰岛素能力下降有关:高胰岛素血症参与了患者睾酮的合成。

The role of insulin in the pathogenesis of polycystic ovarian syndrome

Thirty polycystic ovarian syndrome (PCOS) women witth normal LH or LH/FSH and 15 agematched normal women were studied to investigate the differences in endocrine reponse to oral glucose tolerance test (OGTT). The results revealed: (1) all subjects had normal glucose tolerance despite of the significantly higher glucose levels at 30,60,90 min in PCOS; (2)In PCOS,response to insulin were about 2 to 3 times higher and also higher C-peptide levels,as compared with the normal; (3)serum testosterone level was increased by (1.97nmol/L) at the end of the test while the controls showed the reverse. These findings suggested that hyperinsulinemia, which were related to insulin resistance in peripheral tissues,and decreased hepatic clearance in these patients may be involved in the production of high levels of testosterone.
