腰椎间盘突出症翻修的原因分析和应对方法

目的分析腰椎间盘突出症初次手术失败的原因,探讨翻修的应对方法和疗效。方法对2005-05以来收治的38例腰椎间盘突出症术后复发的原因进行分析,并实施翻修手术。结果翻修的主要原因为初次手术不够彻底、多间隙突出遗漏及术后出现腰椎失稳等。实施翻修手术后优28例,良6例,可3例,差1例。优良率为89.5%。结论对复发的腰椎间盘突出症实施椎间融合,后路椎弓根系统内固定术,可获得满意的疗效。

The cause analysis and strategies of revision surgery of lumbar intervertebral disc herniation

Objective To analyze the reasons of the first failure operation of lumbar intervertebral disc herniation and to explore the coping methods and treatment effect of the revision surgery.Methods The reasons for 38 cases of recurrent lumbar intervertebral disc herniation in our hospital since May 2005 were analyzed,and the revision surgery was implemented.Results The main reasons for the revision surgery were the first operation not thorough enough, many prominent clearance omission and postoperative lumbar spinal instability and so on.Conclusion For recurrent lumbar interbertebral disc herniation implementation of between vertebra posterior fusion and vertebral pedicle fixation system can get satisfactory effect.
