联合检测血清PG、CA19-9、CEA在胃癌诊治中的临床意义

目的：探讨血清胃蛋白酶原（PG）Ⅰ、PGⅡ、PGⅠ/PGⅡ、糖类相关抗原19-9（CA19-9）、癌胚抗原（CEA）在胃癌诊断及预后评估中的意义。方法：应用放射免疫测定法,检测60例胃癌病人和40例正常人的血清PGⅠ、PGⅡ、CA19-9以及CEA的值,并结合临床病理资料对结果进一步分析。结果：胃癌病人血清PGⅠ的值低于对照组,而CA19-9、CEA的值高于对照组,组间差异有统计学意义（P〈0.05）,胃癌病人血清PGⅠ、CA19-9的值与胃癌的不同临床分期有关（P〈0.05）,而血清CEA的水平只在Ⅰ、Ⅱ期与Ⅳ期间的差异有统计学意义（P〈0.05）。胃癌病人血清PGⅠ、CA19-9、CEA的值与胃癌的淋巴结转移有关（P〈0.05）。PGⅠ、PGⅡ和PGⅠ/PGⅡ的AUC分别为0.850、0.502和0.852,表明PGⅠ、PGⅠ/PGⅡ对胃癌的诊断价值较高,而PGⅡ对胃癌的诊断价值较低。结论：联合检测血清PG、CA19-9及CEA能够提高胃癌诊断的准确率,并且对胃癌的早期诊断有一定的临床应用价值。

THE CLINICAL SIGNIFICANCE OF SERUM PG, CA19-9,CEA IN DIAGNOSING GASTRIC CARCINOMA

Objective:To investigate the diagnosis and prognosis value of serum PGⅠ,PGⅡ,PGⅠ/PGⅡ,CA19-9 and CEA level for patients with gastric cancer.Methods:The contents of serum PGⅠ,PGⅡ,CA19-9 and CEA were detected by Radioimmunoassay in 60 patients with gastric cancer and 40 healthy people.Then the results were investigated combined with clinicopathological data.Results:The contents of serum PGⅠin gastric cancer group was significantly lower than the control group,while the contents of serum PGⅠand CA19-9 were higher than the control group(P<0.05).The contents of serum PGⅠand CA19-9 had statistical relatiuity with clinical stage(P<0.05),while the contents of serum CEA only had statistical relativity betweenⅠ,Ⅱ and Ⅳ stage(P<0.05).The contents of serum PGⅠ,CA19-9 and CEA had statistical relativity with lymph node metastasis(P<0.05).The AUC of PGⅠ、PGⅡand PGⅠ/PGⅡwere 0.850、0.502 and 0.852.It showed that PGⅠand PGⅠ/PGⅡ had a higher diagnostic value in gastric cancer,while the diagnostic value of PGⅡwas lower.Conclusion:The combined detection of serum PG,CA19-9 and CEA could improve the accuracy of diagnosing in gastric cancer.They had clinical application value for early gastric cancer diagnosis.
