距骨骨折病例回顾分析

目的 分析距骨骨折的临床流行病学特征,以提高距骨骨折的预防和治疗水平.方法 回顾性分析我院1993年4月～2007年4月收治的急性距骨骨折患者.收集指标：性别、受伤时年龄、致伤原因、是否手术及手术方式、手术时间、住院时间、合并损伤、随访结果.结果 136例患者中,男女比例为7∶1;20～39岁青壮年占54.4%;最常见的致伤原因为高处坠落.在本组病例中,HawkinsⅡ型和Ⅲ型距骨颈骨折及SneppenⅡ型距骨体骨折较为常见,分别占79.6%和57.5%.最常见的合并损伤为同侧踝关节骨折,以内踝骨折最为常见.7例患者行手法复位石膏固定保守疗法,5例患者拒绝手术治疗.其余患者行手术治疗,平均手术时间115min（30～270min）,平均住院时间13d（3～23d）.结论 距骨骨折治疗满意度较差,距骨缺血坏死和创伤性关节炎的发生率较高.

Clinic Epidemiological Analysis of Talus Fractures

Objective To investigate the epidemiological characteristics of talus fraetures, so as to obtain a better knowledge of such injuries,which may help their prevention and treatment. Methods A retrnspeetive study was done on the data of 136 patients with talus fraetures who had been admitted into our department from April 1993 to April 2007. The following data were analyzed：gender,age（when injured） ,causes of injury,type and time of operations,time of hospitalization,associated injuries,therapeutic outcome. Results Of the 136 patients,the ratio of men to women was 7 to 1.54.4% of the patients＇ages were from 20 to 39. The most eommon cause of injury, was falling from a high place. Compared with other type,Hawkins Ⅱ and Ⅲ types of talar neck fracture and Sneppen Ⅱ type of talar body fracture were more often found,the percent was 79.6% and 59.5% respeetively. The most common associated injury was ipsilateral ankle fraeture. Conclusion With high incidence of osteoneerosis of talus body and traumatic arthritis,the degree of satisfaction of talus fraeture treatment was very low.
