老年性脑出血患者继发癫痫的临床探讨

目的研究老年性脑出血患者继发癫痫的发病率、发作类型、出血部位与发作的关系及预后.方法对36例资料完整的老年脑出血后继发癫痫患者进行临床分析.结果老年性脑出血患者继发癫痫发病率为10.8%,早期发作与脑水肿密切相关,多为部分性发作,晚期发作与中风囊、神经元变性及胶质增生等病理刺激有关,多见于脑出血后2周～6月内,以全身强直阵挛性发作为主.脑叶出血后癫痫发作,需系统规则治疗.结论老年性脑出血患者继发癫痫并不少见,其发作与脑水肿、中风囊、神经元变性等多种因素有关.脑叶出血易致癫痫发作,晚期发作需系统规则用药治疗.

The clinical investigation of epileptic seizures following intracerebral hemorrhage in the elderly

Objective To study the incidence rate,type of attack,relationship between hemorrhage postitions and manifestation,and its prognosis of epileptic seizure following intracerebral hemorrhage(ICH) in the elderly.Methods Clinical materials are analyzed in 36elder patients with epileptic seizures following ICH.Results The incidence rate of epileptic seizures following ICH in the elderly patients is 10.8%.Early seizures are related to brain edema,most of which were partial seizures.Late seizures were related to the pathologic stimulation of stroke cyst,neuron degeneration and colloid hyperplasia.Most seizures are between 2 weeks and 6 months after hemorrhage.Generalized tonic-chronic seizures plays an important role in it.Epileptic seizures following lobar hemorrhage must be treated regularly.Conclusion Epileptic seizures following ICH are not rare in elder patients.Seizures are related to brain edema,stroke cyst,neuron degeneration and so on.Lobar hemorrhage leads to epileptic seizures easily.Regular and systematic treatment is needed in the patients with late manifestation.
