CT仿真喉镜临床应用

摘    要:目的探讨CT仿真喉镜(CTVL)的临床应用价值及限度.材料与方法对40例经病理证实的喉及下咽部病变作CTVL成像,并与轴位、纤维喉镜和手术所见对照分析.结果CTVL能显示39例(97.5%)病变,对观察假声带和前联合受侵优于轴位;36例头端CTVL上显示的病变部位、大小和形态与纤维喉镜所见基本一致,4例不如后者;6例足端CTVL所见弥补了纤维喉镜的不足.CTVL不能观察喉内结构的活动和组织的颜色变化、难以判断病变的性质及不易显示病变的深部浸润状况.结论CTVL能很好地显示喉及下咽部病变的部位及侵犯范围,是轴位图像和纤维喉镜的有力补充手段.

Abstract:Purpose: To evaluate the clinical applications and limitations of CT virtual laryngoscopy (CTVL). Materials and Methods: CTVL was performed in 40 patients with larynx and hypopharynx diseases confirmed by pathology. The results were then compared with the findings of axial images, conventional laryngoscopy and operation. Results: CTVL could display the lesion in 39 cases (97.5%), and was superior to axial images in detecting the invasion of false vocal cords and anterior commissure. The location, size and extension of lesions showed on CTVL by cranio caudally were corresponded well with that of conventional laryngoscopy in 36 cases, and 4 lesions were much not as good as laryngoscopy. CTVL demonstrated more information of the lesions by caudocranial approach, and that was inaccessible to fiber optic laryngoscopy in 6 cases. However, CTVL could not display the change of tissue color and the motion of laryngeal structures, and it was difficult to investigate the surface appearances, characterization of lesion and detect the deep invasions. Conclusion: CTVL can clearly demonstrate the location and extension of lesions of larynx and hypopharynx, and is a good complementary method for axial images and conventional fiber optic laryngoscopy.
