支气管哮喘患儿外周血辅助性T淋巴细胞17、白细胞介素-17、白细胞介素-21水平变化

摘    要:目的 检测辅助性T淋巴细胞17(Th17)及细胞因子IL-17、IL-21在支气管哮喘(哮喘)患儿外周血中的表达,探讨其在儿童哮喘发病机制中的作用.方法 以哮喘患儿60例(哮喘组)为研究对象,根据病情分为哮喘发作期(发作期组,n=30)、哮喘缓解期(缓解期组,n=30);以健康儿童30例作为健康对照组.分离外周血单个核细胞,采用流式细胞术检测其Th17细胞百分率.采用ELISA法检测各组IL-17、IL-21表达水平.采用SPSS 13.0软件进行统计学分析.结果 哮喘发作期组患儿外周血中Th17及 IL-17 水平明显高于缓解期组和健康对照组(Pa<0.05);哮喘发作期组患儿血清IL-21表达水平明显低于缓解期组和健康对照组(Pa<0.05);哮喘缓解期组与健康对照组外周血Th17、IL-17、IL-21水平比较,差异均无统计学意义(Pa>0.05).结论 Th17、IL-17通过全身系统性炎症反应参与儿童哮喘的发病,IL-21可作为儿童哮喘的控制指标,亦有望为哮喘的治疗提供新的靶点.

Abstract:Objective To investigate the levels of T helper lymphocyte 17(Th17) cell and interleukin-17(IL-17) cells and interleukin-21(IL-21) in asthmatic children and study their role in pathogenesis of asthma.Methods Sixty children with asthma were divided into 2 groups according to the severity of asthma: the children with acute exacerbation of asthma(n=30),and the alleviated period asthma children(n=30).The healthy children(n=30) were served as healthy control group.Peripheral blood mononuclear cells(PBMC) were isolated,and the percentages Th17 of PMBC were detected by flow cytometry.The levels of IL-17,IL-21 in plasma were detected by enzyme-linked immunosorbent assay.The data were analyzed by SPSS 13.0 software.Results Compared with alleviated period asthma children and healthy children,the expressions of Th17,IL-17 increased in children with acute exacerbation of asthma,and the difference was statistically significant(Pa<0.05),and the expression of IL-21 was decreased(Pa<0.05).Compared with healthy control group,the expressions of Th17,IL-17 and IL-21 had no obvious changes in alleviated period asthma children,the difference was not statistically significant(Pa>0.05).Conclusions Th17,IL-17 cells may play a role in the pathogenesis of childhood asthma as a systemic inflammation reaction.IL-21 can be used as control index of asthma in children,and it is expected to be provide new targets for asthma treatment.
