膝髋骨关节炎患者术后疼痛程度与疼痛自我管理行为的相关性研究

摘    要:目的:研究膝髋骨关节炎患者术后疼痛程度与疼痛自我管理行为及两者之间的相关性。方法:对127例膝髋骨关节炎手术患者于入院时进行基本资料评估;术后24 h、48 h、72 h使用疼痛评估调查表评估患者接受调查时的疼痛强度和过去24 h最痛、最轻及平均疼痛程度;术后3 d使用患者术后疼痛自我管理行为问卷进行调查,并对疼痛程度和疼痛自我管理行为进行相关分析。结果:患者术后接受调查时、最痛、最轻以及平均疼痛程度达到中度以上分别占29.13%、100.00%、3.94%和59.84%。患者术后疼痛自我管理行为总分为(18.99±3.67)分。男性、城镇居民、文化程度高和有手术史的患者术后疼痛自我管理行为评分均高于女性、农村居民、文化程度低和无手术史患者(P<0.05~P<0.01)。患者术后疼痛程度与疼痛自我管理行为评分呈负相关关系(P<0.01)。结论:膝髋骨关节炎患者术后疼痛控制不佳,疼痛自我管理能力较差。在临床疼痛管理中,应重视患者自身的作用,不断提升患者的疼痛自我管理能力,以达到有效镇痛,提高患者生活质量。

Abstract:Objective:To explore the correlation between the postoperative pain degree and pain self-management behavior in patients with knee and hip osteoarthritis. Methods:The data of 127 operation patients with knee and hip arthritis were evaluated at admission. The pain intensities were evaluated using the Pain Assessment Questionnaire after 24 h,48 h and 72 h of operation. The pain intensity at the start of investigation, and the worst pain, lightest pain and average intensity of pain at past 24 h were investigated. The pain self-management behavior questionnaire was used to investigate the pain after 3 d of operation. The correlation between pain intensity and total score of pain self-management behavior was analyzed. Results:The proportions of the current pain,worst pain,lightest pain and average pain,which achieved a moderate degree or above,were 29. 13%,100. 00%,3. 94% and 29. 13%,respectively. The total score of postoperative pain self-management behavior was(18. 99 ± 3. 67). Univariate analysis showed that the scores of postoperative pain self-management behavior in male,urban residents,high education level and history of surgery patients were higher than that in female, rural residents,low education level and no history of surgery patients(P<0. 05 to P<0. 01). The postoperative pain intensity was negative correlation with the score of pain self-management behavior(P<0. 01). Conclusions:The postoperative pain control and pain self-management ability in patients with knee and hip arthritis are poor. In clinical pain management,the patients' own rolen and pain self-management ability should be improved,which can benefit to ease pain and improve the quality of life.
