危重症评分法在小儿手足口病中的应用效果观察

目的:观察手足口病（HFMD）中小儿危重症患儿评分法（PCIS）的应用效果。方法:以PCIS作为基础，同时结合体温及神经系统症状来进行评分，得出“重症患儿小儿手足口病的评分方法”（PCIS-HFMD）。对2014-2015年收治的重症HFMD患儿分别在入院当天进行评分，其中对照组（A组）采用PCIS评分，治疗组（B组）采用PCIS-HFMD评分。并且根据评分的结果采用相应的治疗和监护方法，评估2种评分方法的临床实际应用效果。结果:B组重症HFMD患儿的抢救成功率高于A组（P<0.05），而2组患儿神经系统的后遗症率及病死率差异均无统计学意义（P>0.05）。结论:PCIS-HFMD能较好地对HFMD重症患儿病情危重程度进行评估。在HFMD重症患儿中应用该评分方法，能提高抢救成功率，值得在临床儿科领域推广应用。

Application effect of the Pediatric Critical Illness Score in hand-foot-mouth disease

Objective:To observe the application effects of the Pediatric Critical Illness Score ( PCIS ) in hand-foot-mouth disease ( HFMD) . Methods:The “Pediatric Critical Illness Score of hand-foot-mouth disease( PCIS-HFMD)” was established according to the PCIS combined with the scores of body temperature and nervous system symptom. The severe HFMD children from 2014 to 2015 were divided into the control group ( group A ) and treatment group ( group B ) . The control group and treatment group at admission were scored using PICS and PCIS-HFMD,respectively. The corresponding monitoring and treatment methods were implemented according to the scoring results. The clinical application effects of two kinds of scoring methods were analyzed. Results:The saving success rate in group B was higher than that in group A(P <0. 05). The differences of the neurological sequelae incidence and case fatality rate between two groups were not statistically significant(P>0. 05). Conclusions:The PCIS-HFMD can evaluate the severity of children with severe HFMD. The application of PCIS-HFMD in severe patients can improve the saving success rate,and reduce the incidence of neurological complications and mortality,which is worthy promotion in clinic.
