D-二聚体对伴肝脏疾病的胃癌患者临床预后的预测价值

目的：探讨D-二聚体对伴肝脏疾病的胃癌患者临床预后的预测价值。方法选择55例合并肝硬化的行胃癌根治性手术的患者，分析患者预后的影响因素，重点分析D-二聚体对患者预后的预测价值。结果55例患者的1年生存率为78.2%，3年生存率为43.6%，5年生存率为18.2%。单因素分析显示，D-二聚体水平、年龄、Borrmann分型、病理组织分型、肿瘤大小、pTNM分期、肝功能Child-Pugh分级对患者生存率有影响（P﹤0.05）；多因素分析提示，D-二聚体水平、pTNM分期、肝功能Child-Pugh分级是患者预后的独立危险因素（P﹤0.05）。D-二聚体升高组与D-二聚体正常组比较，pTNM分期及肝功能Child-Pugh分级比较差异均有统计学意义（P﹤0.05）。结论 D-二聚体对预测伴肝脏疾病的胃癌患者的临床预后有一定价值。

Prognostic value of D-dimer in predicting the prognosis of gastric cancer patients with liver disease

Objective To investigate the prognostic value of D-dimer in predicting the prognosis of gastric cancer pa-tients with liver disease. Method 55 cases of gastric cancer combined with liver cirrhosis who were administered with gastrectomy were included in the analysis. analyze the factors influencing the prognosis of patients, while the predictive value of D-dimer was investigated in particular. Result For the 55 patients, the 1-, 3-, and 5-year survival rate was 78.2%, 43.6%, and 18.2%, respectively. And the univariate analysis showed that, the level of D-dimer, age, Borrmann classification, pathology, tumor size, pTNM stage, and Child-Pugh score were associated with patients’survival rate (P<0.05);And multivariate analysis suggested that the level of D-dimer, pTNM stage, and Child-Pugh score were indepen-dent risk factors for prognosis (P<0.05). The pTNM stage and Child-Pugh score were significantly different between pa-tients with increased and normal level of D-dimer (P<0.05). Conclusion D-dimer is of clinical significance in predicting the prognosis of gastric cancer patients with liver disease.
