结核及肺癌所致胸腔积液中MCP-1、RANTES的表达水平及临床意义

摘    要:目的:探讨结核以及肺癌所致的胸腔积液中单核细胞超化蛋白-1(MCP-1)和分泌因子(RANTES)的表达水平及临床意义。方法对74例结核性胸腔积液以及64例肺癌致胸腔积液患者进行回顾性分析。对所有患者胸腔积液中MCP-1、RANTES浓度进行检测并进行比较。结果结核组胸腔积液总蛋白、乳酸脱氢酶(LDH)、腺苷酸脱氨酶(ADA)、癌胚抗原(CEA)与肺癌组相比差异均有统计学意义(P﹤0.05);结核组胸腔积液MCP-1浓度、RANTES浓度分别为(2304.9±294.0)pg/ml、(301.5±61.7)pg/ml,均高于肺癌组(P﹤0.05);不同病理类型肺癌患者胸腔积液MCP-1、RANTES浓度比较,差异均无统计学意义(P﹥0.05)。结论 MCP-1、RANTES浓度对于区分结核与肺癌致胸腔积液有着较高的临床价值,但不能区分肺癌的不同病理类型。

Abstract:Objective To investigate the expression and clinical significance of MCP-1 (monocyte chemotactic pro-tein 1) and secreted factors (RANTES) in pleural effusion of patients with tuberculosis or lung cancer. Method 74 cases with tuberculous pleural effusion (tuberculosis group) and 64 cases with lung cancer-induced pleural effusion (lung can-cer group) were retrospectively analyzed. Patients’pleural effusion was extracted to detect the expression of MCP-1 and RANTES. Result The expression of total protein, LDH, ADA and CEA in tuberculosis group and lung cancer group was significantly different (P<0.05);the concentration of MCP-1 and RANTES in tuberculosis group was (2304.9±294.0) pg/ml and (301.5±61.7) pg/ml, and were significantly higher than those in lung cancer group (both P<0.05);While the concen-tration of MCP-1 and RANTES in patients with different pathological classifications of lung cancer was of no significant difference (P>0.05). Conclusion The concentration of MCP-1 and RANTES is of great importance in the clinical differ-entiation for pleural effusion induced by tuberculosis or lung cancer;Though it may not be applied to distinguish the dif-ferent pathological classifications of lung cancer.
