瞬时弹性成像对肝纤维化的诊断价值

目的:探讨瞬时弹性成像(Fibroscan,FS)对肝纤维化的诊断价值。方法:用Fibroscan测定68例肝脏疾病患者的以肝穿刺活组织病理检查的结果作为有无肝纤维化及肝纤维化程度的标准,对肝硬度值(liver stiffness measurement,LSM)诊断肝纤维化进行受试者工作特征(ROC)曲线分析。结果:Fibroscan诊断有无肝纤维化(S0/S1～4)的敏感性为83.33%(45/54),特异性为62.28%(9/14),与病理检查的诊断符合率为73.53%(50/68)。Fibroscan诊断严重肝纤维化(S3-4/S0-2)的敏感性为63.33%(19/30),特异性为81.58%(31/38),与病理检查的诊断符合率为73.52%(50/68)。Fibroscan诊断肝硬化(≥S4/S0～3)的敏感性为66.67%(8/12),特异性为89.29%(50/56),与病理检查的诊断符合率为85.29%(58/68)。结论:Fibroscan对判断肝纤维化程度具有参考意义。

Diagnostic Value of Fibroscan in Hepatic Fibrosis

To investigate the diagnostic value of Fibroscan （FS） in hepatic fibrosis. Methods： 68 patients were detected by FS and liver biopsy. Liver stiffness measurement （LSM） was used to indicate the liver hardness. Receiver operating characteristic curves （ROC） were constructed to show the diagnostic power of FS. Results.. The sensitivity and specificity of FS was 83.33%（45/54） and 62.28% （9/14） respectively in the diagnosis of liver fibrosis （S0/SI-4） ; the diagnostic accordance rate was 73.53 % （50/68）. When FS was used to diagnose severe fibrosis（S3-4/S0-2）, the sensitivity was 63.33 % （19/30）; the specificity was 81.58% （31/58） ; the diagnostic accordance rate was 73.52% （50/68）. When FS was used to diagnose hepato- cirrhosis, the sensitivity was 66.67% （8/12）; the specificity was 89.29 （50/56） ; the diagnostic accordance rate was 85.29% （58/68）. Conclusions.- FS is valuable for the diagnosis of liver fibrosis and cirrhosis.
