肠内营养治疗阿尔兹海默病临床疗效观察

摘    要:目的:了解老年阿尔兹海默病(AD)患者肠内营养治疗前后营养状况及认知功能改善情况,探讨营养干预对改善老年AD患者认知功能障碍的作用。方法共纳入接受治疗的AD患者64例,随机分为治疗组和对照组各32例。对照组患者口服盐酸美金刚片治疗,治疗组患者在口服盐酸美金刚片基础上增加肠内营养乳剂(TPF-D)口服,观察治疗前与治疗3、6、12个月后两组患者营养和认知功能状况,分析营养干预对改善AD患者认知功能的作用。结果治疗12个月后治疗组AD患者的血红蛋白(Hbg)、清蛋白(Alb)水平及简易营养评估法(MNA)评分改善明显优于对照组,差异有统计学意义(P〈0.05);治疗6个月后,患者简易智能状态量表(MMSE)评分和日常生活能力量表(ADL)评分呈升高趋势,且治疗组ADL评分上升优于对照组(P〈0.05);治疗12个月后治疗组MMSE、ADL评分升高明显优于对照组,差异均有统计学意义(P〈0.05)。结论肠内营养干预治疗AD能有效改善患者营养状况,对AD患者认知功能有一定改善作用。

Abstract:Objective To investigate the changes of nutritional status and cognitive function of the elderly with Alzhermer′s disease(AD) before and after eternal nutrition(EN),and explore the effect of nutritional intervention for improving the cognitive function of the elderly with AD. Methods Sixty-four patients with AD were enrolled and randomized into treatment group and control group,32 cases in each group. The control group was given memantine tablets orally,while the treatment group was added with TPF-D orally on the basis of the control group. To observe nutritional status and cognitive function before treat-ment and 3,6 and12 months after the treatment,and analyze the effect of nutrition intervention on the improvement of cognitive function in patients with AD. Results After 12 months of treatment,haemoglobin B(Hbg),Alb,MNA in AD patients of the treat-ment group was significantly better than that of the control group(P〈0.05);after 6 months of treatment,the scores of MMSE and ADL appeared increasing trend,and ADL score in the treatment group increased significantly than that in the control group (P〈0.05);After 12 months of treatment,the levels of MMSE and ADL in the treatment group increased significantly than those in the control group with statistically significant difference(P〈0.05). Conclusions Nutrition intervention in eternal can improve the nutrition-al status of AD patients effectively,and it has certain effect in improving the cognitive function of patients with AD.
