北京市部分地区的传染病患者在社区卫生服务机构的就诊状况调查

目的了解北京市部分地区的传染病患者在社区卫生服务机构的就诊情况,探讨社区卫生服务机构在传染病防治工作中存在的问题及其改进措施。方法于2009年10月和2010年6月两个时间点,对在北京市四个城区(东城区、西城区、朝阳区、顺义区)所属社区卫生服务机构就诊的传染病患者进行问卷调查。结果在社区卫生服务机构就诊的传染病患者,以慢性疾患如肝炎、结核为主;急性传染病以腹泻为主。患者在病情危重的情况下首选综合/专科医院就诊;但在一般情况下,如果社区卫生服务机构和综合/专科医院合作关系良好,愿意在社区卫生服务机构就诊并留存病案的患者比例明显增加。患者希望通过就诊时医生的讲解和参加系统的传染病知识讲座来获取相关知识;建议社区卫生服务机构提高诊疗水平、与综合/专科医院建立良好的合作关系。结论社区卫生服务机构和综合/专科医院建立良好的合作关系,可以形成有利于患者、社区卫生服务机构以及综合/专科医院的三方互利的模式。

Visits of Patients with Infectious Diseases to Community Health Care Organizations in Beijing

Objective To investigate the visits of patients with infectious diseases to community health care organizations(CHCOs) in Beijing. Methods Patients with infectious diseases visiting CHCOs in four districts of Beijing(Dongcheng,Xicheng,Chaoyang,and Shunyi) in October 2009 and June 2010 were surveyed using a self-designed questionnaire. Results The most common infectious diseases were chronic hepatitis and tuberculosis,whereas diarrhea was the most common acute infectious disease.Most patients preferred to visit a comprehensive or specialized hospital for critical conditions.However,an increasingly large number of patients were willing to visit and file their medical records in a CHCO in case of mild illness if such organizations had established partnership with one or more comprehensive and/or specialized hospitals.The patients were keen to be informed about infectious diseases through consultations and lectures by doctors.They also hoped that the CHCOs had more well-trained medical staff and more stable partnership with higher-level hospitals. Conclusion Good partnership between CHCOs and comprehensive and/or specialized hospitals is beneficial to the hospitals and the patients,and thus may attract/retain more patients to CHCOs.
