基本药物制度对医疗卫生机构经济运行的影响

目的了解基本药物制度对医疗卫生机构经济运行状况的影响。方法运用分层抽样和随机数字表法,在我国东中西部分别选取4个省,每省在首批实施基本药物制度的基层医疗卫生机构中选取2家社区卫生服务中心和3家乡镇卫生院,进行财务收支状况的调查并分析其结果。结果与2009年相比,2010年,调查机构总收入平均增长9.24%,总支出平均增长12.17%,收支结余平均为5.39万元,下降82.75%。平均财政补助收入增长51.24%;医疗收入增长1.01%,药品收入下降23.97%。结论实施基本药物制度后,基层医疗卫生机构总收入仍保持增长趋势,但收支结构发生变化,其中财政补助收入上升,药品收入下降,医疗收入基本持平,收支结余总体下降。

Impact of Implementing National Essential Drugs System on Economic Operation in Primary Health Care Institutions

Objective To investigate the impact of the national drugs system on the condition of economic operation in primary health care institutions. Methods The stratified random sampling and random number table method were taken to choose 2 community health service centers and 3 township hospitals where the national drug policy had been implemented as the first trial institutions form each of 4 Provinces in Eastern,Central and Western China.The conditions in these institutions on financial revenues and expenditures were investigated and analyzed. Results Compared with 2009,in 2010,the total income of the investigated institutions increased averagely by 9.24%,and the total expenditure by 12.17%.The average balance was RMB 53 900 yuan,decreasing by 82.75%.The average government input increased by 51.24%;medical income increased by 1.01%;and drug income decreased by 23.97%. Conclusion After implementation of the national drug policy,the total income in primary health care institutions has still been in a tendency of increase.However,the structure of income and payment has changed:the financial assistance has improved,the drug income has decreased,the medical income keeps a constant basically,and the balance has been generally decreased.
