MRI动态增强扫描联合磁共振胰胆管造影在胆道梗阻性疾病诊断中的价值

摘    要:目的探讨MRI动态增强扫描(LAVA)联合磁共振胰胆管造影(MRCP)对胆道梗阻性疾病的诊断价值。方法选取2010-07-01—2011-05-01我院收治的40例梗阻性黄疸患者,均行LAVA、MRCP检查,对良恶性胆道梗阻的LAVA、MRCP影像学特征进行回顾性分析。结果 LAVA、MRCP检查显示:40例梗阻性黄疸患者中,末端胆管癌13例,肝门胆管癌5例,胰头癌4例,十二指肠癌2例,壶腹癌1例,十二指肠憩室炎1例,胆管结石12例,胰腺导管内乳头状黏液瘤2例。结论 LAVA和MRCP的联合应用对胆道梗阻性疾病的鉴别诊断具有重要价值,其影像学结果对临床治疗决策有指导意义。

Abstract:Objective To investigate the value of magnetic resonance imaging dynamic enhanced scan(LAVA)combined with magnetic resonance cholangiopancreatography(MRCP) in the diagnosis of biliary obstruction. Methods 40 patients with obstructive jaundice admitted to our hospital from July 2010 and May 2011 were tested with MRI and MRCP.The imaging features of benign and malignant diseases were analyzed retrospectively. Results Among the 40 patients,there were 13 patients with distal bile duct carcinoma,5 patients with hilar bile duct carcinoma,4 patients with pancreas head carcinoma,2 patients with duodenal carcinoma,1 patient with ampulla carcinoma,1 patient with duodenal diverticulitis,12 patients with gallstone and 2 patients with IPMN. Conclusion MRI and MRCP could definitely demonstrate the biliary obstruction disease.The imaging results play an important role in differential diagnosis and in estimating the level of the process as well as guiding the treatment.
