输尿管镜下钬激光碎石术与开放手术治疗输尿管结石临床对照研究

目的探讨输尿管镜下钬激光碎石术与传统开放手术(输尿管切开取石术)治疗输尿管结石的临床疗效及其优缺点。方法选取我院182例输尿管结石患者,其中136例患者行输尿管镜下钬激光碎石术,46例患者行开放手术治疗,观察两组疗效、手术时间、术中出血量、住院时间、并发症发生情况以及术后恢复情况。结果两种方法临床疗效相近,但输尿管镜下钬激光碎石术治疗输尿管结石与传统开放手术比较,手术时间短、术中出血量少、住院时间短、术后双J管移位少、胃肠功能恢复早、离床活动早、术后输液时间短、镇痛药物使用少、重返工作岗位时间短。结论输尿管镜下钬激光碎石术是治疗输尿管结石一种安全、有效的理想手术方法,也是输尿管中下段结石治疗的首选方法。

Clinical Study on Urethroscopic Holmium YAG Laser Lithotripsy and Open Ureterolithotomy in the Treatment of Ureteral Calculi

Objective To investigate the clinical effect,advantage and disadvantage of urethroscopic holmium YAG laser lithotripsy and open ureterolithotomy.Methods 182 patients with ureteral calculi were involved into the study.136 patients were given urethroscopic holmium YAG laser lithotripsy and 46 patients were given open ureterolithotomy.The operative effect,operative time,bleeding,hospital stay,complication and postoperative recovery were compared between the two groups.Results The clinical effects of the two methods were pretty much the same,but compared with open ureterolithotomy,ureteroscopic holmium YAG laser lithotripsy had shorter operation time,less bleeding and hospital stay,early recovery of intestinal function,shorter time of lying in the bed and transfusion,less use of pain reliever and early returning to work.Conclusion The urethroscopic holmium YAG laser lithotripsy is safe and effective in the treatment of ureteral calculi and should be the first choice when treating middle and lower ureteral calculi.
