新诊断2型糖尿病患者血清脂肪甘油三酯脂酶及抵抗素与胰岛素抵抗的关系研究

摘    要:目的探讨新诊断2型糖尿病(T2DM)患者血清脂肪甘油三酯脂酶(ATGL)、抵抗素的表达水平及其与胰岛素抵抗的关系。方法选取新诊断T2DM患者55例(T2DM组)及健康体检者30例(正常对照组)。检测两组受检者空腹血清ATGL和抵抗素水平,并计算其胰岛素抵抗指数(HOMA-IR)、胰岛素敏感性指数(ISI)。对AT-GL、抵抗素及其他因素进行单因素相关分析,然后进行多元逐步回归分析。结果 (1)T2DM组患者血清ATGL水平〔(23.59±3.94)μg/L〕显著低于正常对照组〔(31.20±2.05)μg/L〕,而血清抵抗素水平〔(8.97±5.73)μg/L〕显著高于正常对照组〔(7.41±1.88)μg/L〕,差异有统计学意义(P<0.01)。(2)单因素相关分析显示,血清ATGL与ISI呈正相关(P<0.01),与空腹血糖(FPG)、体质指数(BMI)、HOMA-IR、抵抗素呈负相关(P<0.05)。血清抵抗素与FPG、BMI、HOMA-IR呈正相关(P<0.05),而与ISI呈负相关(P<0.01)。(3)多元逐步回归分析发现,HOMA-IR、ISI作为独立危险因素影响ATGL、抵抗素水平。结论血清ATGL、抵抗素可能在胰岛素抵抗的形成中具有重要的作用,可能参与了T2DM发生、发展。

Abstract:Objective To explore the relationship between fatty triglyceride lipase(ATGL) and resistin and insulin resistance in patients with newly diagnosed type 2 diabetes mellitus(T2DM).Methods 55 T2DM patients and 30 cases with healthy volunteers(NC group) were selected.Levels of fasting plasma ATGL and resistin were detected,then calculated the insulin resistance index(HOMA-IR) and insulin sensitivity index(ISI),analysed the correlation between ATGL,resistin,and other factors.Results(1) Plasma ATGL((23.59±3.94)μg/L) in T2DM group were significantly lower than that in the NC group((31.20±2.05)μg/L)),and the plasma resistin((8.97±5.73)μg/L) were significantly higher than that in the NC group((7.41±1.88)μg/L),difference were statistically significant(P<0.01).(2)Single-factor correlation analysis showed that serum ATGL was positively correlated with ISI(P<0.01),and was negatively correlated with FPG,BMI,HOMA-IR,and resistin(P<0.05);And serum resistin was positively correlated with FPG,BMI,HOMA-IR(P<0.05),while was negatively correlated with ISI(P<0.01).(3)Stepwise multiple regression analysis found that the HOMA-IR and the ISI,as independent risk factors,affected the levels of ATGL and resistin.Conclusion Serum ATGL and resistin likely play an important role in the formation of the insulin resistance,may be involved in T2DM occurrence and development.
