血清抵抗素与老年2型糖尿病大血管病变的相关性研究

目的探讨抵抗素与老年2型糖尿病(T2DM)及其大血管病变和相关代谢指标的关系。方法将133例T2DM患者分为大血管病变组(70例)和非大血管病变组(63例),并选50例老年健康者作为对照组。测定3组受试者空腹血清抵抗素、游离脂肪酸(FFA)及超敏C反应蛋白(hs-CRP)水平;并测定各组的空腹血糖(FPG)、空腹胰岛素(FINS)和血脂水平;用胰岛素抵抗指数(HOMA-IR)评价胰岛素抵抗程度,分析各指标间的相关性及其与大血管并发症的相关性。结果大血管病变组及无大血管病变组患者血清抵抗素、FFA及hs-CRP水平均显著高于对照组,差异有统计学意义(P<0.05)。相关分析发现,老年T2DM患者血清抵抗素水平与腰臀比(WHR)、hs-CRP、FFA、HOMA-IR、三酰甘油(TG)呈正相关(P<0.05),与高密度脂蛋白胆固醇(HDL-C)呈负相关(P<0.05)。以抵抗素为因变量,进行多元线性逐步回归分析,HOMA-IR及WHR进入回归方程。结论老年T2DM患者血清抵抗素水平升高,提示血清抵抗素与老年T2DM的发病及其大血管病变的发生有关。

Study on the Relationship between Serum Resistin and Vascular Disease in Elderly Type 2 Diabetic Patients

Objective To investigate the relationship between serum resistin,type 2 diabetes mellitus,its macrovascular disease and relative indexes in elderly people. Methods 133 T2DM patients were divided into macrovascular disease group(70 cases) and non macrovascular disease group(63 cases),and 50 healthy elderly were selected as control group.Fasting serum resistin,free fat acid(FFA) and hypersensitivity CRP levels of the three groups were detected.FPG,FINS and blood lipid levels were also detected in each group.HOMA-IR was used to assess insulin resistance level and the relationship between each index and their correlation with macrovascular disease were analyzed. Results The levels of serum resistin,FFA and hs-CRP in macrovascular disease groupand non macrovascular disease group were significantly higher than those in the control group(P<0.05).Correlation analysis showed that the elderly T2DM serum resistin concentration was positively correlated with WHR,hs-CRP,FFA,HOMA-IR and TG(P<0.05),but negatively correlated with HDL-C(P<0.05).With resistin as the dependent variable,multivariate linear regression analysis was performed,and HOMA-IR and WHR entered the regression equation. Conclusion The elderly T2DM patients have elevated serum resistin levels,suggesting that serum resistin is correlated with T2DM occurrence in elderly as well as macrovascular disease.
