Livin和Caspase-3在散发性大肠管状腺瘤癌变过程中的表达及相关性研究

摘    要:目的研究凋亡抑制基因Livin在散发性大肠管状腺瘤癌变发生、发展中的可能作用,并探讨其与Caspase-3的关系。方法应用免疫组化法检测118例散发性大肠管状腺瘤伴不同程度异型增生及癌变的组织中Livin和Caspase-3的表达情况,同时选取25例正常大肠黏膜作对照。结果 Livin在正常大肠黏膜组、大肠管状腺瘤伴异型增生组及癌变组的阳性表达率分别为16.0%(4/25)、57.3%(51/89)和86.2%(25/29),大肠管状腺瘤伴异型增生组和癌变组的Livin阳性表达率明显高于正常大肠黏膜组(P<0.05),且癌变组的Livin阳性表达率高于大肠管状腺瘤伴异型增生组(P<0.05);大肠管状腺瘤伴轻度、中度、重度异型增生组Livin阳性表达率分别为50.0%(15/30)、54.5%(18/33)和69.2%(18/26),差异无统计学意义(P>0.05)。大肠管状腺瘤伴异型增生组和癌变组Caspase-3阳性表达率分别为40.4%和10.3%,均明显低于正常大肠黏膜组(76.0%,P<0.05),且癌变组的Caspase-3阳性表达率明显低于大肠管状腺瘤伴异型增生组(P<0.05);大肠管状腺瘤伴轻度、中度、重度异型增生组Caspase-3阳性表达率分别为66.7%(20/30)、39.4%(13/33)和11.5%(3/26),差异有统计学意义(P<0.05)。Livin和Caspase-3的表达呈负相关(r=-0.305,P<0.05)。结论 Livin可能下调Caspase-3的表达,共同促进大肠管状腺瘤癌变的发生。Livin和Caspase-3可能是大肠管状腺瘤癌变的重要调控基因。

Expression of Livin and Caspase-3 in the Carcinogenesis of Sporadic Colorectal Tubular Adenoma and Their Correlation

Abstract:Objective To study the possible role of Livin and Caspase-3 in the carcinogenesis of sporadic colorectal tubular adenoma and their correlation.Methods The immunohistochemical staining was used to detect the expression of Livin and Caspase-3 in 118 patients with sporadic colorectal tubular adenomas combined with different levels of dysplasia as well as cancerous changes,and 25 normal colorectal mucosa as control.Results The positive expression rate of Livin was 16.0%,57.3% and 86.2% respectively in normal colorectal mucosa group,colorectal tubular adenoma with dysplasia group and colorectal tubular adenoma with cancerous changes group.The positive expression rate of Livin was significantly higher in the cases of colorectal tubular adenoma with different dysplasia group and colorectal tubular adenoma with cancerous changes group than that in normal colorectal mucosa group(P<0.05),and the positive expression rate of Livin was significantly higher in colorectal tubular adenoma with cancerous changes group than that in colorectal tubular adenoma with different dysplasia group(P<0.05).The positive expression rate of Livin was 50.0%(15/30),54.5%(18/33) and 69.2%(18/26) respectively in the cases of colorectal tubular adenoma with mild,moderate and severe dysplasia,but it showed no statistically significant difference(P>0.05).The positive expression rate of Caspase-3 in colorectal tubular adenoma with dysplasia group and colorectal tubular adenoma with cancerous changes group was 40.4% and 10.3% respectively,both significantly lower than that in normal colorectal mucosa group(76.0%,P<0.05).The expression rate of Caspase-3 was significantly lower in colorectal tubular adenoma with cancerous changes group than that in colorectal tubular adenoma with dysplasia group(P<0.05).The positive expression rate of Caspase-3 was 66.7%(20/30),39.4%(13/33) and 11.5%(3/26) respectively in the cases of colorectal tubular adenoma with mild,moderate and severe dysplasia,and it showed statistically significant difference(P<0.05).Negative correlation was found between the expression of Livin and Caspase-3(r=-0.305,P<0.05).Conclusion Livin might down-regulate Caspase-3 expression and promote adenocarcinoma.Livin and Caspase-3 may be important controlling genes in the carcinogenesis of colorectal tubular adenoma.
