维持性血液透析患者血小板凝血酶受体的表达及其意义

摘    要:目的观察维持血液透析患者(研究组)血小板蛋白酶激活受体1(PAR1)、蛋白酶激活受体4(PAR4)和糖蛋白Ibα(GPIbα)的表达及其与正常对照者(对照组)之间的差异。方法采用流式细胞术检测两组的PAR1,PAR4和GPIbα的表达,采用比浊法检测其血小板最大聚集率(MAR)。结果维持性血液透析患者血小板GPIbα表达略低于对照组,但差异无统计学意义(P=0.072);血小板PAR1表达高于对照组,血小板MAR低于对照组,差异均有统计学意义(P<0.05);相关性分析显示PAR1的表达和血液透析频率之间存在明显正相关(r=0.601,P<0.05)。结论与正常对照者相比,维持性血液透析患者的血小板PAR1表达明显升高,而PAR1的高表达又可引起血小板的进一步活化,从而使血液的高凝状态加剧,可能增加动脉粥样硬化及血管意外发生的概率。

Abstract:Objective To observe the difference of platelet membrane receptors proteinase activated receptor-1(PAR1),proteinase activated receptor-4(PAR4) and glycoprotein Ibα(GPIbα) in maintenance haemodialysis(HD) group and in the normal control group. Methods We used flow cytomery to test the expression of PAR1,PAR4 and GPIbα and used turbidimetric method to test the maximal platelet aggregation rate(MAR). Results HD group being compared with control group,the expression of GPIbα was a little lower,but the difference was not significant(P=0.072);The expression of PAR1 was higher and the platelet MAR was lower,all with significant difference(P<0.05).The correlation analysis concluded that there was a positive relationship between the expression of PAR1 and the frequency of haemodialysis(r=0.601,P<0.05). Conclusion The expression of platelet PAR1 in HD patients obviously went up,which resulted in further activation of platelet.As a result,patients′ blood would become more and more coagulated and the probability of atherosclerosis and vascular accidents may be greatly increased as well.
