妊娠合并心脏病64例临床分析

目的 :探讨妊娠合并心脏病对孕产妇及新生儿的危害 ,心功能分级与新生儿体重的关系。方法 :回顾性分析我院 1995年至 2 0 0 0年 6 4例妊娠合并心脏病患者 ,其心功能分级与新生儿体重的关系 ,心功能衰竭的发病时间 ,以及剖宫产在妊娠合并心脏病的意义。结果 :本组 6 4例妊娠合并心脏病中 ,心肌炎、有症状的预激综合征、围产期心肌病等引起的心律失常居第一位 ,占 4 2 .2 % ,其次为风湿性心脏病 ,占 2 9.7% ,第三为先天性心脏病 ,占 2 3.4 % ;心功能衰竭发于孕 32～ 34周者占 6 1.1% ;剖宫产率占 84 .4 % ,剖宫产中无孕产妇及新生儿死亡 ;低体重儿发生率随心功能分级增高而增加 ,新生儿平均体重随心功能分级增高而减少。结论 :在妊娠合并心脏病中 ,妊娠合并风湿性心脏病所占比例明显下降 ,妊娠合并心律失常所占比例升为第一位 ,妊娠合并先天性心脏病所占比例明显上升 ;孕32～ 34周为心功能衰竭多发期 ,剖宫产可以降低孕产妇及新生儿死亡。

Retrospective analysis of 64 cases of pregnancy complicated by heart disease

Objective: To study the endangerment of heart disease of pregnant women on outcome of their pregnancies. Methods: Clinical data of 64 cases of pregnant women with heart diseases from 1995 to 2000 were analyzed retrospectively. Results: The highest incidence of heart disease was arrhythmia, account for 42.2%; the second incidence was rheuma-heart disease, account for 29.7%, and congenital heart disease was on the third, account for 23.4%, 61.1%of heart failure occured in pregnant 32 weeks to 34 weeks. The rate of casesrean birth was 84.4%. The higher the grade of heart function in women, the higher the incidence of low weight of newborns, but the lower the average weight of newborns. Conclusions: The incidence of arrhythmia and congenital heart disesase were increasing. Heart failure more common occur in pregnant 32 weeks to 34 weeks.
