十二指肠乳头括约肌小切开后大球囊扩张在治疗胆总管结石中的应用

目的回顾性分析十二指肠乳头括约肌小切开后大球囊扩张治疗胆总管结石的有效性与安全性。方法 138例MRCP证实小于20mm的胆总管结石行小切开后以8～20mm球囊扩张后取石。结果完全取石率95.7%（132/138）,术后并发症胰腺炎10.1%,胆管炎1.45%,胆囊炎2.17%,无出血及死亡病例。结论十二指肠乳头括约肌小切开后大球囊扩张治疗胆总管结石安全有效,且保留了括约肌功能。

Small Endoscopic Sphincterotomy plus Large-balloon Dilation in Treatment of Choledocholithiasis

Objective A retrospective evaluation of the efficacy and safety of small endoscopic sphincterotomy（SEST） plus large-balloon dilation（LBD）（8 ～ 20 mm balloon diameter） in treatment of choledocholithiasis.Methods 138 cases with common bile duct stones（the maximal diameter was less than 20 mm） confirmed by MRCP were performed with LBD after SEST for retrieval of stones.Successful stone removal and complications such as pancreatitis and bleeding were main outcome measurements.Results Stone clearance was successful in 132 patients（95.7%）,complications occurred in 16（11.6%）,including pancreatitis 14（10.1%,mild grade12,middle grade 2,no severe case）,cholangitis（1.45%）,cholecystitis（2.17%）,no bleeding and death cases.Conclusion The SEST plus LBD is safe and effective procedure for CBD stones.
