心肌肌钙蛋白T检测对小儿急性病毒性心肌炎的诊断价值

目的探讨心肌肌钙蛋白T(cTnT)对小儿急性病毒性心肌炎的诊断意义。方法对102例疑似小儿急性病毒性心肌炎患儿分别于入院时即发病(4.5±3.8)d,抽取周围静脉血同时测定cTnT和CK-MB水平,观察小儿急性病毒性心肌炎这两项指标的阳性率。结果 cTnT和CK-MB在小儿急性病毒性心肌炎患儿就诊时的阳性率分别为94.3%、95.5%。两组间的差异无统计学意义(P>0.05)。cTnT检测急性病毒性心肌炎的特异性(92.2%)优于CK-MB(71.4%),其敏感性(94.3%)低于CK-MB(95.5%)。结论在诊断小儿急性病毒性心肌炎时,cTnT检测方便、快速,特异性高,对小儿急性病毒性心肌炎的诊断具有很好的价值,联合检测cTnT及CK-MB,可提高诊断率,减少漏诊率。

Diagnostic value of Cardiac troponin T(cTnT )in children with acute viral myocarditis

Objective To explore diagnostic significance of cardiac troponin T(cTnT) in children with acute viral myocarditis.Method We collected venous blood samples from 102 cases suspected to be acute viral myocarditis at the time of admission(4.5±3.8 days after incidence),examined concentration of cTnT and CK-MB,to detect the positive rates of these two indicators in children with acute viral myocarditis.Result The positive rate of.cTnT and CK-MB in children with acute viral myocarditis are 94.3%,95.5% respectively,and there is no significant difference between the two groups(P >0.05).The specificity for cTnT(92.2%) to indicate acute viral myocarditis within children is superior to CKMB,while its sensitivity(94.3%) lower than that of CK-MB(95.5%).Conclusion cTnT indicates good diagnostic value for acute viral myocarditis within children,for the detection of it is convenient,rapid,and with high specificity.A combined detection of cTnT and CK-MB can improve accurately diagnostic rate,reduce the rate of misdiagnosis.
