术中颅脑CT检查对迟发性外伤性颅内血肿的诊治价值

目的探讨术中CT检查对迟发性外伤性颅内血肿的诊治价值。方法10例颅内血肿患者均在第一次开颅术后带气管插管行颅脑CT检查,发现对侧迟发性颅内血肿,立即行迟发性颅内血肿清除术。结果术后恢复良好9例,死亡1例。结论在清除一侧急性外伤性颅内血肿时,若患者出现脑膨出,对侧瞳孔散大,术前对侧有脑挫裂伤或小血肿应带气管插管行颅脑CT检查,尽早发现对侧迟发性颅内血肿,尽早手术,以提高抢救成功率。

CT Check during Operation in Treating Delayed Traumatic Intracranial Hematoma

Objective To explore the experience of CT check during operation to treat delayed traumatic intracranial hematoma in the opposite side.Methods The clinical data of 10 cases were analysed retrospectively that the CT check were performed during operation and discovered opposite tardive intracranial hematoma.Results Among the 10 patients with delayed traumatic intracranial hematoma who accepted treatments,9 cases recovered well,1 case died.Conclusion When one side of acute traumatic intracranial hematoma is removing,encephalocele,opposite pupil dilatation occurs on the patients.The patients with contusion and laceration of brain or small hematoma in the opposite side before operation should be checked on the head by CT under the condition of tracheal intubation.To find opposite tardive intracranial hematoma and perform operation as early as possible can increase the achievement ratio of rescuing patients.
