摘    要:目的 探究维持性血液透析(MHD)患者的氧化应激水平,并分析其影响因素,为干预提供依据.方法 选取MHD患者152例(HD组),选取健康人群30例(对照组),分别检测各组的血浆丙二醛(MDA)、总抗氧化能力(TAC)、C反应蛋白(CRP)、血浆白蛋白(ALB)、肌酐(Scr)、尿酸(UA),并统计MHD患者的透析龄、是否应用静脉铁剂、促红细胞生成素(EPO)用量等信息,运用多重线性回归法分析微炎症状态、透析龄、血浆白蛋白、肌酐等因素对MHD患者氧化应激状态的影响.结果 HD组的MDA水平高于对照组(P<0.05),而TAC水平低于对照组(P<0.05),微炎症状态、静脉补铁、透析龄、Scr与MDA呈正相关,其中微炎症状态的标准化偏回归系数最大.ALB、UA、EPO与MDA呈负相关、与TAC呈正相关.结论 MHD患者的氧化应激水平高于正常人群,且微炎症状态对氧化应激的影响高于其他因素.

Multivariate analysis of oxidative stress in maintenance hemodialysis patients
Abstract:Objectives To investigate the oxidative stress in maintenance hemodialysis(MHD)patients and analyze the multiple factors of oxidative stress.Methods A total of 152 patients receiving hemodialysis were selected in this study,and 30 healthy volunteers were selected in the control group.Malondialdehyde(MDA),total antioxidant capacity(TAC),serum creatinine (Scr),C reactive protein (CRP),Uric Acid (UA) and albumin (ALB) were detected between the two groups.Meanwhile,the related information of MHD patients was selected,such as dialysis ages、dosage of Erythropoietin (EPO) and so on.The data were analyzed by multiple linear regression.Results The level of MDA in MHD patients was higher than that in control group(P <0.05),and the level of TAC in MHD patients was lower than that in control group(P < 0.05).For MHD patients,microinflammation,use of intravenous iron,dialysis ages and Scr were positively correlated with MDA.Moreover,the standardized partial regression coefficient of microinflammation was the largest among all factors.ALB,UA and EPO were negatively correlated with MDA and positively correlated with TAC.Conclusions The oxidative stress in MHD patients are more severe than that in healthy people.Microinflammation is the most influential factor on oxidative stress of MHD patients.
