嗅沟脑膜瘤的显微外科治疗

目的 探讨嗅沟脑膜瘤的手术方法及效果。方法 回顾性分析36例嗅沟脑膜瘤的临床资料和随访结果。结果 冠状切口进路24例。单侧额下进路12例；肿瘤全切除34例，分期全切除2例；死亡1例。术前原视力下降的19例，术后均有不同程度的恢复。原有精神障碍的9例，术后6例逐渐好转。3例精神障碍程度减轻。25例术后随访半年至8年，有3例在术后3年、5年、7年复发。结论 根据病人的临床表现，结合CT、MRI检查可明确诊断；显微手术切除肿瘤是嗅沟脑膜瘤最佳的治疗方法。

Microsurgical Treatment of Olfactory Groove Meningiomas

Objective To explore the effect of microsurgical therapy on olfactory groove meningiomas. Method A retrospective analysis of the clinical data of 36 cases of olfactory groove meningiomas was made. Results The operation through bifrontal approach was performed in 24 patients and through unilateral subfrontal approach in 12. Of 36 patients with olfactory grove meningiomas, 34 underwent total resection of the tumors and 2 received total resection of the tumors by stages. One patients died 7 days after the operation. Nineteen patients who had visual disturbance before the operation were significantly improved in their visual acuity after the operation. Of 9 patients with mental disorder before the operation, 6 were recovered and 3 improved in mental symptoms after the operation. In 3 of 25 patients followed up from 6 months to 8 years, the tumors recurred 3, 5 and 7 years respectively after the operation. Conclutions CT and MRI are important methods to confirm the diagnosis of olfactory groove meningiomas. Microsurgery is the best way of treating olfactory groove meningiomas.
