腹腔镜手术治疗卵巢良性肿瘤(附76例报告)

目的 :总结卵巢良性肿瘤腹腔镜手术治疗的效果。方法 :回顾分析 76例卵巢良性肿瘤行腹腔镜下肿瘤剥除术、切除术的临床资料。结果 :76例良性肿瘤腹腔镜手术成功率 75例 (占 98 6 8% ) ,中转开腹1例 ,手术切口均Ⅰ期愈合 ;平均住院 3 2± 1 4d ;术后出血 5例 (占 6 5 8% ) ;发生皮下气肿 7例 (占 9 2 1 % ) ;2例分别在术后 2、4个月患侧卵巢良性增大。结论 :腹腔镜手术治疗卵巢良性肿瘤创伤小、恢复快、安全有效 ;彻底、完整剥除肿瘤囊壁是预防复发的关键 ;适时中转开腹可降低并发症的发生。

Treatment of the benign ovarian tumor by laparoscopy

Objective:To summarize the experience of laparoscopic surgery treatment benign ovarian tumor.Methods:The clinical data of 76 laparoscopic surgery treatment cases were retrospectively analyzed. Results:The operations were successful in 75 cases(98.68%),1 case was converted to laparotomy.All operative incisions were primary healing.The mean duration of hospitalization was 3.2±1 4 days;Hemorrhage rate was 6.58% (5/76).7 cases had subcutaneous emphysema (9.21%);2 cases happened ovarian benign enlargement in affected side. Conclusions:Laparoscopic surgery treatment benign ovarian tumor has advantages of mininal invasion,quick recovery,safety and effective operation results.Thorough and intact stripping of tumor cystwall is the key to prevent to recurrent.It is important that correctly predominate the opportunity of immediate laparotomy can decrease complications.
